<abstract>
<title>Abstract</title>
<p>We present a case report of an antenatal mother who was diagnosed with COVID-19. The psycho-social stressors experienced by her and the psychological intervention provided to her through telephone have been documented. The importance of post discharge telephone support or tele-psychiatry during the pandemic is also elucidated in this paper.</p>

<title>INTRODUCTION</title>
<p>WHO defines health as a collective component of physical, mental and social well-being of an individual.<xref ref-type="bibr" rid="B1"><sup>1</sup></xref> COVID-19 is a pandemic that has proven to be a major stressor worldwide. Apart from the medical complications of the illness, it has significant impact on the psychological and social aspects of an individual&#x2019;s life. It has been found that children, older adults, pregnant women, person with chronic medical conditions, immunocompromised individuals are more vulnerable to infection.<xref ref-type="bibr" rid="B2"><sup>2</sup></xref> There are psychosocial stressors during pregnancy and it could lead to adverse outcome to the baby like preterm birth.<xref ref-type="bibr" rid="B3"><sup>3</sup></xref> It is quite important to have positive mental health at this time as it ensures the person has resources to manage the ongoing stressors related to pandemic. Here we share a case report on COVID positive pregnant lady and various psycho-social stressors experienced by her during the COVID illness.</p>
<sec>
<title>Case Report</title>
<p>Ms. N, aged 26 years, diagnosed as COVID positive was an antenatal mother currently at 38 weeks, (G<sub>2</sub> P<sub>2</sub> A<sub>0</sub> L<sub>1</sub>) at the time of admission in Chengalpattu Medical College and Hospital (CMCH) for further management. She was noted to be crying frequently and hence was referred to Department of Psychiatry for an opinion on her mental status. A detailed assessment was carried out. Following a detailed interview, it was found that she had the following psychosocial issues:</p>

<list-item><p>She was upset because her husband and their two-year-old child were placed in a different hospital and was worried when a relative of her&#x2019;s had mentioned that her child had been left unattended without care.</p></list-item>
<list-item><p>She kept ruminating about the potential sources of her infection - she had celebrated her first&#x2019;s son&#x2019;s birthday at home privately but later had gone to the temple for seeking blessings from the priest. She had also visited a crowded grocery store and later she had undergone an ultrasound scan</p></list-item>
<list-item><p>She had interpersonal relationship issues with her mother in-law as it was a marriage of choice and there were problems due to different social backgrounds and class differences between families. She, along with her husband and child had moved out to a rented home a month ago as her in-laws&#x2019; home was dilapidated and she had wished to set up a new home of their own.</p></list-item>
<list-item><p>She was upset as her sister started avoiding her phone calls when she was found to be COVID positive. Her sister had stated that police and revenue officials would be questioning her unnecessarily and hence had told her not to call her.</p></list-item>
<list-item><p>Her next concern was how her neighbors would react towards her when she got back to her rented home, as the authorities had already approached her house owner. Her house owner and five of his family members had been placed in a government hospital nearby for observation.</p></list-item>
<list-item><p>She was also alone in a huge isolation ward which made her feel anxious.</p></list-item>

<p>In view of these issues, the following interventions were suggested and carried out:</p>
<list list-type="order">
<list-item><p>It was decided to shift her husband and their child to the same hospital premises where she was, so that she felt comfortable with their proximity.</p></list-item>
<list-item><p>She was moved to a room closer to the nursing station so that there would be options for better monitoring and care.</p></list-item>
<list-item><p>A call was made to her sister and she was educated about her condition and also explained the need for her ongoing support to the client through phone.</p></list-item>
<list-item><p>Planned to call her neighbors to alleviate their concerns and to make the environment more conducive for her return.</p></list-item>

<p>The very same day, it was noted that there was foetal distress, and hence an emergency LSCS was carried out. The mother and baby were safe during and after the LSCS. On follow up rounds, it was found that she was able to sleep well and appetite was adequate. She remained in touch with her husband and first child through phone. She was able to take care of herself and also her new born baby well. The baby was by her side in the cradle and she continued breast-feeding. She appeared calm and happy all throughout her stay in the post-delivery ward.</p>
<p>At the time of discharge, she had mixed emotions of excitement to be with the family as well as anxiety related to how the extended family and neighbor-hood would react. After a week of discharge, we got in touch with her through telephone support services. It was found that she was asked to vacate her rented home immediately. It was with despair that she went to be in her in-law&#x2019;s home again. She was taught basic coping strategies and provided supportive psychotherapy through the phone. After a month from discharge, she was contacted again through our tele-psychiatry service and it was found that she had gotten adjusted to her situation. She appreciated our telephone support services and mentioned that she felt supported right from the day of admission till date.</p>
<p>She insisted that the service need to be provided to other patients in need during the pandemic.</p>
</sec>
</sec>
<sec sec-type="discussion">
<title>DISCUSSION</title>
<p>This is a case report about a 26 years old multigravida female, who acquired COVID infection during the third trimester of her pregnancy. The main psychological stressor for her was the separation from her husband and child. This practice of separating mother from her family members for COVID-19 protection has harmful impacts on maternal health.<xref ref-type="bibr" rid="B4"><sup>4</sup></xref> Another concern of her&#x2019;s was about the stigma prevailing in her own family members and general public because of COVID-19. Shaming and blaming the affected people can be hurtful which further deteriorates their mental health.<xref ref-type="bibr" rid="B5"><sup>5</sup></xref> Family stressors include the interpersonal conflict with her in-laws. Interpersonal issues and depression have a dose-response relationship. More the conflict more is the probability of experiencing prenatal depression.<xref ref-type="bibr" rid="B6"><sup>6</sup></xref> We suggested shifting her family members to the same vicinity to reduce the stress of separation. Attempts to reduce the stigma in her sister and her neighbors were initiated through phone calls. A guide to combat the stigma due to ongoing pandemic was developed by UNICEF, WHO, IFRC. This guide could create awareness about do&#x2019;s and dont&#x2019;s among public during this pandemic period.<xref ref-type="bibr" rid="B7"><sup>7</sup></xref></p>
<p>Owing to fetal distress, she had to undergo an emergency caesarean section. WHO advice is that caesarean sections should only be performed when medically justified.<xref ref-type="bibr" rid="B8"><sup>8</sup></xref> The newborn baby was kept in close contact with her and early exclusive breastfeeding was initiated with proper respiratory hygiene. According to WHO there is no restriction for a COVID positive mother to breastfeed her baby, because only breastfeeding helps the baby to thrive and prevent infections.<xref ref-type="bibr" rid="B8"><sup>8</sup></xref> There is no available literature addressing the psychosocial stressors of COVID-19 patients in the post discharge period. Our patient&#x2019;s primary concern was about the reaction of the extended family, neighbor-hood and living situation which were addressed through our telephone support service and it provided relief to her. We strongly feel based on our experience, that, there is a pressing need to identify COVID positive patients who are experiencing bio-psychosocial stressors and continue to provide post discharge mental health support through telephone support or tele-psychiatry services, as it builds up their morale and help them to cope with the situation much better.</p>
</sec>
<sec sec-type="conclusion">
<title>CONCLUSION</title>
<p>We recommend telephone support or tele-psychiatry service for COVID 19 patients in need of psychosocial support, at the time of admission, during hospital stay and post discharge.</p>
